W21C studied five dimensions of social vulnerability and how these may predict post-discharge adverse outcomes. The five social dimensions looked at were financial constraints, transportation barriers, social supports, health literacy and health benefits and priority of health in the midst of other life circumstances. The findings from this study were used to inform interventions aimed to lower the risk of readmissions for medical patients being discharged from hospital.
