Path to Home (P2H) was a patient-flow initiative designed to reduce hospital wait times and over-crowding in Alberta hospitals. By coordinating services and setting performance targets, the aim was to make each patient’s path into the hospital, and then back home, as efficient as possible. W21C researchers studied Path to Home (which has since been incorporated into an even larger AHS initiative) as an instance of organizational change through a rigorous qualitative study. The results from this study will help in the design and implementation of future health care innovations.
