About Us

The Canadian Hospital at Home Community is a national network of clinicians, researchers, health leaders, and policymakers advancing the development, implementation, and financial as well as environmental sustainability of Hospital at Home (HaH) programs across Canada. Their mission is to foster innovation, collaboration, and knowledge exchange to improve patient care, reduce hospital pressures, and transform the way acute care is delivered.

What is Hospital at Home?

Hospital at Home (HaH), also known as Hospital in the Home (HITH) is an innovative model of care that delivers hospital-level treatment (including staff, equipment, technologies, therapies) to patients in the comfort of their own home. It is a safe, effective, patient-centered alternative to traditional inpatient hospitalization for selected individuals with acute medical conditions that substitutes for acute care days in hospital.

Definition: What is Hospital at Home?

HAH is:

  • Episodic care (not chronic disease management)
  • 24/7 medical oversight
  • Daily virtual or in-person physician care
  • In-home nursing, diagnostics, and therapies
  • Remote monitoring technologies
  • Rapid response pathways for escalation

HAH IS NOT:

  • Primary care
  • Skilled nursing facility care
  • Home care
  • Outpatient parenteral therapy
    Virtual only care
  • Hospital prevention outpatient care

The GLOBE-HaH Tool is a global initiative designed to define, compare, and analyze Hospital at Home (HaH) programs. It was collaboratively developed for HaH programs that provide acute hospital-level substitutive care at home, as defined by WHAHC 2023.
