Evaluation of The Connect 2 Care (C2C) Team for Vulnerable Patients with Complex Needs

Socially vulnerable individuals with unstable housing often have complex health problems that lead to high acute care utilization, poor health outcomes, and high associated costs. In order to better care for these vulnerable populations, Calgary Urban Project Society (CUPS), Alpha House, and the O’Brien Institute at the University of Calgary have partnered to implement and evaluate the C2C program which is a mobile outreach team that provides care coordination for low-income and homeless patients who seek care in the emergency department or hospital. W21C is supporting the robust program evaluation to ensure the program’s goals are achieved of improving patient care and health outcomes, patient and staff experiences, and reducing acute care use and costs.
